BH Intake Clinician

General Position Description:††The Intake Clinician is responsible for all aspects of the initial assessment of the member and their family. The initial assessments are completed for intake into appropriate services, including: gathering data for a comprehensive psychosocial assessment, making clinical diagnosis and completing the service plan. Intakes performed may be for both adults and children. The Intake Clinician may provide direct client services consisting of case management services to individuals and/or family members. This position may also be involved in crisis services as appropriate or necessary. The individual in this position must meet all established guidelines and requirements including the timelines for referrals and written documents. The individual also must successfully complete all required training identified by the RBHA and EBF and meet the credentialing and privileging requirements as identified by AHCCCS.

Essential Duties and Responsibilities (With or Without Accommodations):

In an office, home and/or community based setting as appropriate the Intake Clinician:

  • Receive DCS referrals and make contact with childrenís placement within 24 hours.
  • Complete the Rapid Response assessment within 72-hours of referral.
  • Conducts a comprehensive psychosocial assessment, summarizes primary presenting issues, formulates diagnosis, and makes treatment recommendations.
  • Provides effective crisis assessment.
  • Completes all necessary paperwork related to enrolling an individual into the Easter Seals Blake Health Home.
  • Refers members to appropriate community resources as necessary.
  • Provides Individual and Family Therapy as requested and appropriate.
  • Attend and participate in team decision making meetings.
  • Attend and participate in Preliminary Protective Hearings.
  • Complete progress notes in HIMS within 24 hours of service.
  • Communicate with DCS, placement, biological parents and any other team member to initiate/coordinate services.
  • Respond to crisis calls and performs other additional services, as needed.
  • Engages families and offers services, as appropriate during the initial admissions process.
  • Attends and participates in Child and Family Team and Adult Recovery Team meetings as needed/required.
  • Attends and participates in required staff meetings.
  • Submits accurate and complete written reports within required timelines as identified by the RBHA and AHCCCS.
  • Complete extensive clinical documentation in client-centered manner
  • Develops and maintains positive relationships with program participants, staff, other programs and state agencies.
  • Maintains agreed upon work schedule, is punctual, keeps appointments, and schedules/uses time effectively.
  • Uses supervision effectively, accepts feedback in a professional manner, and accepts responsibility for job performance.
  • Communicates essential information effectively with EBF team staff and team members accurately as well as in a timely manner.
  • Understands and maintains confidential information within the confines of HIPAA.
  • Performs any other duties as assigned and appropriate for the position.
  • Responsible for meeting productivity and documentation requirements set by the supervisor.
  • Complete member intake evaluations, including diagnosis and determination of medical necessity for admitting member to mental health outpatient treatment services.
  • Assigning new clients to appropriate treatment teams and scheduling first appointment
  • Communicating with referral sources or other treatment providers regarding alternative and/or additional services if necessary (i.e. crisis services, other community based resources or providers, substance abuse evaluation, etc.)

Minimum Educational Requirements:

  • Intake Clinician I:†Has a bachelor's degree in a field related to behavioral health; or has a bachelor's degree and at least one year of full-time behavioral health work experience; or has an associate degree and at least two years of full-time behavioral health work experience; or has a high school diploma or high school equivalency diploma and 18 credit hours of post-high school education in a field related to behavioral health completed no more than four years before the date the individual begins providing behavioral health services and two years of full-time behavioral health work experience, or four years of full-time behavioral health work experience.
  • Intake Clinician II:†Has a masterís degree in a field related to behavioral health.
  • Intake Clinician III:†Is licensed at the associate or independent level with Arizona Board of Behavioral Health as a LAC, LMSW, LASAC, LAMFT, LPC, LCSW, LISAC, or LMFT.
  • Valid Arizona driverís license and the willingness to use personal vehicle in the course of employment.† Must meet minimum requirements of agency insurance carrier.
  • Must be able to meet training and agency compliance requirements for the position.
  • Spanish speaking preferred but not required.

Minimum Requirements:

  • Must be at least 21 years of age and able to obtain a Level One Fingerprint Clearance Card.
  • Valid Arizona driverís license and the willingness to use personal vehicle in the course of employment.† Must meet minimum requirements of agency insurance carrier.
  • Must be able to meet training and agency compliance requirements for the position.
  • Bilingual in Spanish preferred, but not required.
