Supported Living Direct Support Professional (Safford)

Department: Blake Employment & Community Living
Location: Safford, AZ

General Position Description:

The Supported Living Program will operate year-round on a seven day a week schedule.  The Supported Living Direct Support Professional will work an agreed upon schedule.  The position works with individuals in a community-based setting.

Supported Living Direct Support Professionals work directly with people in their homes and in the community, assisting them to develop a sense of identity, self-worth and independence.

Essential Duties and Responsibilities:

  • Develop and support a variety of community activities and experiences providing opportunities for adults to assume and maintain valued roles and relationships.  Show innovation and insight in defining roles; develop community connections/partnerships.
  • Demonstrate self-initiative and follow through in carrying out responsibilities in setting up activities to provide maximum participation by each person.  Show creativity and flexibility in daily routines; develop new and different teaching opportunities when possible.  Be willing to adapt to changes and to take on responsibilities without direction.
  • Assist individuals with personal care and hygiene needs.  This includes assistance in the restroom, assistance with eating, and demonstrating proper carrying and lifting techniques (using one-person and mechanical lift transfers.)
  • Encourage and maintain active teamwork.  Deal with problem solving in a professional, non-hostile manner.  Offer options and solutions, and give feedback in a constructive manner.  Report any concerns to your supervisor.
  • Assist individuals with the following areas: meal prep, grocery shopping, bill pay, transportation, social gatherings, medication management and upkeep of home.  Provide this through innovation, taking daily chores/duties and turn them into interesting individual learning experiences.
  • Assist in community activities, plan and conduct incredible activities and everyday fun.

Function as an enthusiastic, competent, contributing member of EBF.

  • Attend and participate in required staff meetings
  • Promote and follow practices that support health and safety of members and employees.
  • Develop and maintain positive relationships with individuals we serve, team members, families and state agencies.  Encourage positive team work and problem solve in a non-hostile manner
  • Complete paperwork within required timelines
  • Attend all necessary training within the time lines established at the time you were hired
  • Write in "in-house" communication log after every shift worked. Maintain and monitor daily communication log between day program and Supported Living program
  • Maintain agreed upon work schedule, be punctual, and use time effectively
  • Use supervision effectively, accept feedback in a professional manner, and accept responsibility for job performance
  • Communicate essential information with EBF team leads and supervisors accurately and in a timely manner
  • Understand and maintain private and sensitive information within the confines of need-to-know parameters.


  • Ability to work flexible hours including early mornings afternoon and evening hours and at least 1 weekend day;
  • Ability to speak and write effectively in English.

  • An ability to acknowledge, respect, and build upon the diversity of person served and their families with regard to such factors as race, ethnicity, family constellation, gender, socioeconomic status, values, and traditions.
  • An ability to teach others using anticipatory guidance, modeling, coaching, reflection, and role-play 
  • Ability to complete job responsibilities autonomously without daily oversight

Minimum Requirements:

  • Age: Must be at least 18 years of age Experience: Any combination of training and experience needed to provide direct care services.
  • If position requires driving an EBF vehicle, must possess a current/valid Arizona Driver’s license and be insurable under the agency’s automobile policy
  • Ability to work flexible schedules: weekends, nights, days, evenings
  • Maintain current Level One Clearance Card.
  • Maintain current CPR, First, Aid, Article IX.
  • Must successfully clear a criminal background check and a Child Protective Services/Department of Children, Youth and their Families central registry check. All newly hired employees are subject to the E-Verify Employment Eligibility Verification Program.

Physical requirements:

The position provides direct caregiving and support for individuals with developmental disabilities. The position requires the following physical requirements:

  • Must be able to utilize Personal Protective Equipment (PPE), such as face mask, gown, goggles and gloves, for any amount of time necessary to provide a physical barrier to protect the employee and others.
  • Must be able to walk, run, climb, stoop, kneel, sit, stand, reach, stretch, bend, squat, and other physical activities.
  • Must be able to move to support individuals with developmental disabilities.
  • Must be able to operate wheelchair brakes, vehicle lifts, etc.
  • Must be able to communicate with individuals with developmental disabilities.
  • May work in outdoor weather work conditions.
  • Must be able to wear PPE, such as a face masks and disposable gloves, to provide a physical barrier to protect you and others as needed

  • Must be able to remain calm, non-defensive with a supportive attitude during stressful, potential crisis, and crisis situations.
  • Must be able to manage high volumes and unpredictability of work.
  • Must be able to remain flexible and reprioritize according to the changing demands of the day.
  • Must be able to manage stress in a fast - paced, unpredictable, and high intensity setting.
  • Must be able to adapt to diverse cultural environments and communicate in a culturally inclusive manner.
  • Must be able to represent EBF in a professional manner in all situations.
